Application for Short & Long Term Missions. This is a pdf form you can complete online. Save a
completed form for your files. Attach a completed form to an email
addressed to missions@shmius.org.

Release Once
completed, this form must be notarized.

Bring these forms with you in your carry on! They may
be required at any time during your trip.

Consent
for Medical Treatment A completed, notarized
and signed Consent for Medical Treatment form for you and each team
member. You may be asked to present these forms in case a member of
your team requires immediate, emergency medical attention at any time
during your trip. Please
note we require a photocopy of each team member's passport to
accompany his or her notarized Consent for Medical Treatment form.

Legal
Consent for a Minor The Legal Consent for a
Minor form will need to be completed, notarized and signed for each
minor on your team and carried with you as your team enters and
departs Mexico. You may be asked to present these forms before
entering and while leaving Mexico. Please note that the Legal Consent
for a Minor form must be completed and signed by BOTH parents,
especially if the parents are separated or divorced.
